  • How to sign up for a sport

    What is required:
    • A physical that is within 365 days of completion
    • Cardiac assessment portion must be signed by your doctor
    • The online sports packet must be completed for each season of participation

    • Soccer
    • Basketball
    • Wrestling
    • Ski Racing
    • Baseball
    • Lacrosse
    • Volleyabll
    Testing will be done after teams are formed and tryouts are over.
  • How to submit a physical

    To: Parents/Guardians of Students Planning on Participating in Sports

    As we approach another sports season, I want to clarify the process by which students are deemed eligible to participate in sports.
    Goals
    1. Your son should have a current physical exam (no greater than 365 days), on the right forms, with all of the proper signatures, dates, and physical exam findings. It should be sent to the Health Office. It can be given to the Athletic Trainer in my absence. Dates of physicals are shared with Athletic Trainer on an ongoing basis.
    2. You should follow the instructions given by the Athletic Trainer, Marcin Szorc, on how to fill out the information he requires online. Marcin has sent everyone an email with instructions and a link to the site(s) that needs to be completed. If you have questions re this requirement, call Marcin at X115.
    A few added notes:

    -if the health office has a current physical exam for your son, you need not send another. If you are unsure of physical exam date, you can either call the Health Office, or ask the Athletic Trainer. The physical exam should indicate date exam was done, NOT the date forms filled were out (if it is another date).

    -if you need a copy of the physical exam, you can download it from the school’s website. Go to donboscoprep.org, sign in as a parent and go to section called RESOURCES. Here you will find HEALTH OFFICE/FORMS. Look to the right hand side of page for the 4 page PRE PARTICIPATION PHYSICAL. These are the only forms accepted.

    -while I am instituting a new online system of form collecting called MAGNUS, if you would still prefer to hand the physical exam to me, scan it to my email, or fax it to me, that will be fine. All paper copies of the physical exam, that have been previously submitted, that are filled out properly and have current dates, are valid.
    -make sure that doctor has signed and dated the Completed Cardiac Assessment Professional Development Module section on the Clearance page.
    -there should not be any outstanding sports or gym restrictions still in effect.

    All of these requirements must be completed before try outs. Please try to hand in paperwork at earliest convenience, to give me ample time to review.

  • Reporting Injuries

    All athletic injuries must be reported to the athletic trainer within 24 hours. Please email Jonathan Gorman - athletic trainer at jgorman@donboscoprep.org or stop in the athletic training room in Immaculata Hall.

    If an injury is sustained over the weekend the student must see the Mrs. Brooks RN - school nurse firstt thing in the morning.
